Enrollment in this hearing program, the vision program, and prescription drug program are included with your American Ceramic Society membership. Members may print the enrollment form (PDF) and mail it directly to the Benefits Processing Center (include $5 shipping/handling charge). An enrollment form will also be enclosed in your new member packet. Your request will be processed and you will receive your identification card in approximately 2 weeks. Present your ID card to the Beltone hearing specialist prior to receiving services.
